Observation

(2)(a) Small cars were accessible in the downstairs playroom. Push pins in the wall were accessible. A bottle cap was accessible in the bathroom. The cap was removed during the site visit. Small choking hazards were accessible in the backyard, some of which were removed during the site visit. (2)(b) Window blinds in the kitchen which is accessible to children had internal cords which can form loops. (2)(c) Electrical cords were accessible in the den, living room and in a side yard. (2)(d) Plastic bags and gloves were accessible in the bathroom. A plastic bag was accessible in the living room and was removed during the site visit. (3)(c) Two stacks of concrete blocks were accessible in the backyard. (3)(d) The doorknobs to the office and laundry room could not be opened from the outside and posed entrapment hazards. The egress window wells on the side of the house were not covered and posed entrapment (& falling) hazards. (3)(e) A sprinkler head was a tripping hazard by the concrete patio in the backyard. (3)(f) A shelf in the downstairs play room was not anchored. A tall shelf in the den was not anchored. (4)(c) A free standing floor lamp was not anchored in the living room. (4)(f) Stairs throughout the multi-level home did not have gates on them. (5)(a) Electrical outlets in the accessible dining area, living room and kitchen were not properly covered. (5)(b) Outlets by the bathroom sink and kitchen sink were not tamper resistant/GFCI type.
